Accessing Our Services - Referral Process  Minimize

Individuals who are seeking services from the Canadian Mental Health Association Grand River Branch contact the Centre for Mental Health in their area.  (Click here for maps and locations)  The Referral Coordinator will contact the person requesting support within two weeks of receiving a referral or request.

After  consulting with the individual who is requesting support, the Referral Coordinator completes the first portion of the Support Services Plan.  This includes the expressed needs of the individual and an indication of the support that is being requested.   The individual will be assigned to a Support Coordinator (dependent upon the wait list) after the Referral Coordinator has completed the intake documentation.

The Support Coordinator contacts the individual to develop a plan to meet the support request within one week of being assigned to an individual.  The Coordinator works with the person to undertake additional planning and recommend a 6-month support plan.  The Support Plan is a brief outline of the support to be provided and is detailed with time-lines.  Everone who receives support services from Canadian Mental Health Grand River Branch requires a Support Services Plan.

 Outreach to Hospital Facilities

CMHA Support Coordinators visit in-patient facilities on a weekly basis and meet with the prime nurses, social work department or psychiatrists about potential referrals. A CMHA designated Support Coordinator may also attend patient/staff meetings and follow up with hospital staff regarding planning and discharge.
