Outreach Services (Available in Cambridge and Guelph)
Outreach Services assist in the identification and support of persons with severe and persistent mental health issues who would not seek services on his/her own and who, without some intervention, would be in serious jeopardy. People receiving Outreach Services are often homeless or socially/geographically isolated, do not seek services on their, nor do they have the natural supports that would seek out such services.
Outreach Support Coordinators identify the needs and offer support and service to people in a non-intrusive way using a "continuous relationship approach" which is critical to establishing the trusting meaningful relationship. Outreach staff respect and work within the values of the "street culture" and offer services where people are such as soup kitchens, shelters, drop-ins etc.
