Support Services available through the Centres for Mental Health range from intermittent support to ongoing intensive Recovery support as needed.
We offer Outreach, Short-Term and Long-Term intensive support services in various locations.
We provide individualized, flexible and holistic support for individuals over 16 years of age who deal with a significant mental health issue or concurrent disorder (mental health and substance use issues).
The duration and intensity of the services will reflect individual needs. Where possible, we will move the support from our service system to the natural support system in the community (families, friends, neighbours, employers etc.) and as necessary, provide support to the person and/or their personal support network.
Supports include:
- Recovery planning
- Person centred and individualized support
- Enhancement of an individuals' ability to pre-determine availability of resources and support in the case of personal crisis by assisting the person to create a self-determined Recovery Support Plan (Crisis Plan).
- Assistance to overcome barriers that Individuals may encounter in accessing services and opportunities in the community.
- Advocacy
- Connection to employment/educational services
- Mental Health & Justice situations
- Enhancement of the individual's informal support network (e.g. family, friends).
- Information, referral and support to the family members and significant others of individuals we support.
